Out of Network

We are on Out of Network Provider and do not accept insurance. A SuperBill is provided that you can present to your insurance for out of network benefits. We suggest you contact your insurance provider to inquire about your benefits. Read more here.

Medicare Statement

This practice has opted out of Medicare and only provides services to Medicare clients under a private agreement. Therefore, Medicare will not be billed for covered therapy services; clients are fully responsible for the cost of psychotherapy services and may not submit practitioner’s fees to CMS for reimbursement. Fees for licensed mental health professional services are not subject to Medicare limits, and MediGap will not pay for the services. Read more here.

Privacy and Health Records

DeepWaters LPC LLLP is committed to protecting the privacy and security of client information. DeepWaters is subject to HIPAA and other applicable federal and state privacy laws, including the Texas Medical Records Privacy Act and Texas mental-health confidentiality law when those laws apply.


Our Notice of Privacy Practices explains how health information may be used and disclosed, your privacy rights, and how to contact the Privacy Officer. The current Notice of Privacy Practices is available on this website and upon request.

Requesting Your Health Records

You may request access to or a copy of your health and billing records as provided by applicable law. Requests should be made in writing.


The preferred way to submit a records request is through the SimplePractice secure Client Portal. You may also contact DeepWaters LPC LLLP regarding a records request by:

Because ordinary email is not a secure method for sending sensitive clinical information, please do not include detailed health information in an email. We may direct you to the secure Client Portal to complete or transmit a records request.


Please identify the records you are requesting and whether you want to inspect them, receive a copy, receive a treatment summary when appropriate, or have records sent to another authorized person or provider.


DeepWaters will respond as promptly as circumstances require and within the time required by applicable law. For Texas mental-health-record requests governed by Texas Health & Safety Code Chapter 611, DeepWaters will act no later than the 15th calendar day after receiving the written request.


Access to a record may be limited or denied only when permitted by applicable law. If access is denied in whole or in part, DeepWaters will provide any notice required by law and will explain any applicable right to review the decision. Ownership of the physical or electronic record system does not limit a client's legal rights of access.


Parents, guardians, minors, personal representatives, and participants in couples or family therapy may have different rights of access depending on applicable law, who consented to treatment, custody or court orders, and the circumstances of treatment.


DeepWaters LPC LLLP is the designated records custodian/entity. If the therapist dies, becomes incapacitated, closes the practice, or transfers the practice, DeepWaters LPC LLLP will maintain or arrange for custody, secure retention, lawful disposition, and access or transfer of records in accordance with applicable law and the practice's records-disposition plan.

You may also file a privacy complaint with the U.S. Department of Health and Human Services Office for Civil Rights or, when Texas law applies, with the Texas Attorney General Consumer Protection Division. DeepWaters will not retaliate against you for filing a complaint or exercising a privacy right.
